Appointment Scheduling Policy


As a Cultivate Community member enrolled in one of our subscriptions, you will be able to connect with our team to set appointments for a variety of services as outlined in your subscription agreement.
In order to set up an appointment, please send an email, including the following information:
  • your name
  • the service or product requested
  • the waypoint location requested
  • at least two requested appointment times

A few important notes!
  • Appointment requests via text or phone will not be accepted - sending them to our email address helps us make sure we don’t miss any of your communications, and that we reply in a timely manner.
  • If you can provide more than two requested times, it is more likely that we will be able to meet one of your requests.
  • Note that at least five business days notice is required for an appointment. For example, if you want to set up an appointment for Monday, send us an email no later than Wednesday morning of the previous week. This way, we’ll have everything you need by the time you meet with us!
  • Remember that masks are required when in the Cultivate suite at Colfax, and when indoors at the Lafayette property. Thank you for helping to keep our community safe.

Our team will provide you with confirmation of the appointment via email, or propose alternative times if neither of your requests can be met.

Remember that some of our subscription members receive unlimited 24-hour access to some practitioner services. If you are interested in changing your subscription, let us know!



Business mailing address and electronic mail service
  • Avoid using your home address for public documents - our mail service provides a mail and package delivery point for you and your business.
  • Business mailing addresses are available through membership packages.   If you are interested in joining as a member, please let us know!

In-house laboratory, per-use
  • If you wish to use the microscope, centrifuge, or other in-house laboratory equipment, process laboratory samples or use our laboratory pickup box, please follow our Waypoint Practitioner Appointment Scheduling Policy.
  • There is a per-use laboratory fee of $35. This allows us to find a time that works for your schedule and provide access to the lab!

Send-out diagnostic laboratory tests
  • If you wish to submit a test to a diagnostic laboratory (e.g. Idexx, Zoetis Diagnostic Labs) under our account, a 100% markup will be applied to all tests.
  • Remember that Cultivate members receive negotiated laboratory test pricing with a 25% markup, and do not pay per-use fees. If you are interested in joining as a member, please let us know!

Wholesale pharmacy
  • All pharmacy items (vaccines, preventative, prescription medications) are available at a standard non-member markup of 100%.
  • Please follow the Waypoint Practitioner Appointment Scheduling Policy in order to schedule medication pickup! There is no additional charge for a pickup appointment.
  • Remember that Cultivate members have access to these products at a member markup rate of 25%. If you are interested in joining as a member, please let us know!

Treatment Room Rental
  • We offer our Treatment Room rentals at the Lafayette waypoint on a per-hour rental basis ($25/hr). Treatment room rental is available during business hours.
  • Please book at least one week in advance. One hour of desk use is included with each hour of treatment room rental so you have plenty of time to complete your record after an appointment. Just remember to reserve the desk if you plan to use it!
 
Desk Use Rental
  • We offer our open office area on a per-hour rental basis ($10/hr).
  • Treatment room rental is available during business hours.
  • Please book at least one week in advance. One hour of desk use is included with each hour of treatment room rental so you have plenty of time to complete your record after an appointment. Just remember to reserve the desk if you plan to use it!

We’ll invoice you on a monthly basis for any amenities used during that month.
Remember that we do offer membership-based pricing! Our members do not pay per-use laboratory fees, receive pharmacy items & lab tests at a markup of 25%, and have access to treatment room & desk use rental discounts. Please let us know if you’d like to see a copy of the membership pricing details.

Please note that all visitors to Cultivate's communal spaces are required to adhere to our Waypoint Co-Working Biosecurity Protocols. Please remember that these are shared spaces for our entire Cultivate community!
